IMPACT-MED

IMPACT-MED aims to design, develop, and deploy an innovative and scalable framework of integrated health solutions for people-centred Multidisciplinary-Cardio-Metabolic-Care (MCMC), supported by a clinically enriched digital ecosystem, combining and offering mature (TRL>8) and innovative (TRL>5) technologies and related services. This will allow to establish a productive and proactive public-private collaboration breaking the fragmentation of care and of siloed solutions, thus avoiding that they are institution-driven or industry-driven, and eventually enable people-centred continuum of care focused on patient’s capacity (low, moderate, hight) in suffering, coping and managing conditions such as hypertension, coronary artery disease, heart failure, diabetes, obesity, atrial fibrillation, and heart valve disease.

ΙΤΙ-CERTH is leading the WP entitled “Digital ecosystem for data integration and AI-enabled solutions”, which will be the technical backbone for the IMPACT‑MED project. This builds a secure digital ecosystem supporting real-time data aggregation and analysis, establishes interoperability between various sources of information (EMRs, RPMs, digital tools), and integrates advanced AI components for predictive analytics and decision support. ITI-CERTH further leads tasks for risk management and quality assurance, as well as data management and IPR.
